Fixed Prosthetics Rx
The 3DL laboratory prescription. Doctor info, patient info, case instructions, materials, restoration type, design & esthetics. Send this with every crown, bridge, veneer, inlay/onlay, or implant restoration.
Remake Request
Use this if a case needs to be remade — interproximals open or tight, occlusion off, margin issue, wrong shade, de-bond, or other. Include the previous case number and ship all parts and screws with the return for implant cases.
